It is pretty unclear, but Tony is actually wearing boy clothes in the first panels. Doesn't mean he isn't scared out of his mind of (Mr) Dr Bernt, fair or not.

In panel 4, Dr. Bernt, Julie, declares that Toni is " fit enough to leave this hospital ". Actually, that is just not true, since Toni has just been severly beaten and probably in no condition to travel. If fact, Toni can not even walk by herself, as seen by the need of the nurse to help her out to the car. But Dr. Bernt is acting in Toni's best interest and removing her to another, safer place.

I wish more doctors, medical practitioners, health care professionals, and medical workers would have that kind of loving concern, and courage to defy the bureaucratic health care system in order to provide the best of care for their patients. The thug who abused Toni, with his justification of " I have morals (yeah, right, and pigs can fly !) ", has probably been working at the hospital for years, and been doing what he has been doing with little or no concern by the management. I have seen bad situations like that many times in my life.
