Eating disorders like anorexia and bulimia really mess with people's brains. One thing people with EDs have in common with trans people is body dysmorphia/dysphoria. The main difference is trans people objectively recognize their bodies, want something different, and get psychological relief when they transition, whereas folks with EDs are often unable to objectively view their bodies no matter how thin they get. Add in the effects of nutritional deficits caused by self-starvation, and the brain wiring misfires in all kinds of (sometimes unfortunate) ways.
